2024-2028 Routt County Public Health Improvement Plan: Coming Soon
Every five years, local public health agencies in Colorado are required to conduct a community health assessment and develop a new public health improvement plan based on the assessment results. This requirement is a result of the Colorado Public Health Act of 2008 and creates a way to address leading public and environmental health issues in the county. Routt County Public Health is excited to share our new 2024-2028 Public Health Improvement Plan with the community. Please stay tuned as this webpage will be updated with the 2024-2028 Routt County Public Health Improvement Plan when it becomes available.
